“Edward” is a 35-year-old Hispanic male presenting with a new rash on the buttocks which has been present for approximately 1 month.
“Edward” is a 35-year-old Hispanic male presenting to my practice with a new rash on the buttocks which has been present for approximately 1 month. He was seen at an urgent care clinic and was prescribed betamethasone-clotrimazole cream used twice daily for 2 weeks, but reports that the rash has only been getting worse. He reports that the areas are very itchy and are starting to spread. Edward denies any history of similar rashes in the past.
When examining Edward’s skin, I note annular, erythematous, scaly plaques with central clearing and well-defined, serpiginous, raised borders as well as erythematous papules and pustules on the bilateral buttocks. He denies any tenderness to palpation. Upon inspection, there are no other areas affected on the body. After questioning, the patient admits to frequent sauna use.
